One of the most common questions I get is how long it takes to see wrinkles smooth out after starting tox. The honest answer: you'll see the muscles relax within a couple of weeks, but the deepest smoothing happens over time.
I'm Amy Hawkins, FNP-C, the injector behind 29 Aesthetics in Bluffdale, Utah. Here's how tox actually works, what to expect after your first appointment, and why consistency makes all the difference.
Tox (like Botox) isn't a magic eraser. It works beneath your skin by blocking the signals between your nerves and muscles. That stops the repeated contractions that cause lines to form, like squinting, frowning, and raising your brows.
The toxin itself doesn't directly smooth out wrinkles. What it does is give your skin the time it needs to recover. As the muscles relax, the skin over them gets a break from the constant folding and creasing that created the lines in the first place.
Over time, that break allows your skin to gradually smooth out. With consistent treatments, lines continue to soften and your skin keeps a smoother, more refreshed look.
Take a look at the before and after photo below.

In the after photo, tox has relaxed the muscles, but the lines are still faintly visible. That's completely normal. Lines that have been etched in for years don't disappear overnight. With consistent treatments, they'll keep softening as the skin heals.
The toxin starts working on the muscles right away. You may not see much change at first, but it's already beginning to relax those muscles and soften the lines.
Within about 7 to 14 days, the full effect kicks in. Lines around your eyes, on your forehead, and between your brows start to smooth out as the muscles become less active. Results happen gradually, which is what gives you a natural, refreshed look instead of a frozen one.
Tox results typically last about three to four months. As the muscles slowly regain movement, it's time to book your next treatment to keep results consistent.
With repeated treatments, your skin can gradually stay smoother. Relaxing the muscles consistently helps prevent fine lines from deepening into permanent wrinkles, and existing lines keep softening the longer they're given a break.
Tox relaxes the muscles, but healthy skin does the rest. If you want to speed up how quickly lines soften, retinol is one of the best additions to your routine. It boosts skin cell turnover and renewal, helping the skin repair faster while the muscles are relaxed.
The retinol I recommend is Alastin's Renewal Retinol, used at night. Daily sunscreen matters just as much, since sun damage breaks down the collagen that keeps skin smooth. For the full routine, see [the skincare routine I swear by as a Bluffdale, Utah injector](link to routine post).
For deeper lines that remain after several rounds of tox, treatments like microneedling can help rebuild collagen and improve skin texture alongside your injections.
Most people start to notice a difference within a few days, with full results at around one to two weeks.
Tox relaxes the muscle, but lines that have formed over years need time to soften. With consistent treatments, they continue to fade as the skin recovers.
Results typically last about three to four months, depending on the treatment area, your metabolism, and how strong the muscles are.
Tox doesn't permanently erase wrinkles, but regular treatments can keep lines from deepening and allow existing lines to soften over time.
A check-in around two weeks after your first treatment is a good idea. That's when you can see your full results and fine-tune anything if needed.
